Simple!
Start off with a 11 x 5.5 inch paper.
Score at 4.25 inch. But PERFORATE at 8.5 inch.
Fold in.
Decorate!
I used a darling apple brad from Eyelet Outlet and
embellished it with a rhinestone
for the top of my book mark.
